What to Expect on Your First Visit
Because Dr. Liddle’s work is highly precise, and because no two mouths are alike, he believes in developing a personalized treatment plan for every patient. On your first visit, Dr. Liddle will meet with you personally to discuss your goals concerning your oral health. The interview will be followed by a thorough examination to gauge your present oral condition.
Based on the interview and examination, Dr. Liddle will develop a custom treatment plan which will best achieve your goals for long-term oral health. It’s important to us that you feel secure in your dental decisions, so Dr. Liddle will present your custom treatment plan with these objectives in mind:
Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Occasionally, treatment can be done the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.
Please assist us by providing the following information
at the time of your consultation:
Your referral slip and any x-rays if applicable.
A list of medications you are currently taking.
Please bring the necessary completed New Prosthodontic Patient Form, 438k (PDF).
This will save time and allow us to help you process any claims.
IMPORTANT: All patients under the age of 18 must be accompanied by
a parent or guardian at the consultation visit.
Please alert Dr. Liddle’s office if you have a medical condition that may be of concern prior to surgery (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).
